Connection Contract
There is power in vulnerability, strength in numbers, and safety in visibility.
Accountability = to be regularly answerable for each of the key areas in your life to qualified people.
PureHeart Ministries respects and values each person that steps forward for partnering. This is a serious and lasting relationship that should glorify God. The enemy would like to see PureHeart and you fail.

It is important that all parties involved are committed to this relationship and see it as a benefit and a priority throughout the week. You should commit to attend each meeting and spend time reading and praying on your own. Your PureHeart mentor will be praying for you.

Confidentiality
PureHeart Ministries is committed to providing individuals a safe place in which they can grown in their full knowledge and understanding of God. One key element to this environment is confidentially.

In general, all information that you discuss with staff and volunteers of PureHeart Ministries will remain confidential and will not be released without your permission. There are some rare circumstances when this information may be disclosed without your permission as required by law. A few examples of these kinds of situations are as follows 1) someone is in danger of being harmed; 2) child/elder is abused or neglected; 3) court order.

PureHeart Ministries’ staff and volunteers may participate in a process where selected individuals are discussed with others for review or consultation. In these situations the dynamics of the problems and issues are discussed, but any information that could be used to determine one’s identity is not. Prayer request are often shared in this manner with prayer partners across the country.

PureHeart Ministries asks that you respect the privacy of others involved in PureHeart Ministries by not sharing what is discussed in small group settings with those outside the small group.

Release of Liability
I have voluntarily initiated and entered into a relationship with PureHeart Ministries. I am under no obligation or compulsion to accept the advice or counsel I may receive during this relationship. I further agree to hold PureHeart Ministries and its staff and volunteers free from any and all liability, loss, or damage of any kind that may arise as the result of this relationship.

I understand that this counsel is not a medical, psychiatric, psychological or other professional service, but is exclusively the sharing and explaining of principles set forth in the Bible as applied to my personal situation.

Acknowledgment
I verify that I have read, have been given an opportunity to ask questions, and understand this informed consent document. I agree to abide by the terms outlined in this document.
